How do doctors determine you have ADD? (attention, deficit, disorder)?

Michelle J asked:

I really really think that I have ADD because I have lots of the signs, and I have an unbelievable hard time focusing like all the time. I’ve been like that my whole life. I want to get tested so I could get medicine to help me with focussing but am afraid that I could end up just not having it and then I will still just have a hard time being focussed. So how do doctors determine whether or not you have it? Is it a brain thing? Or is it whether or not you qualify with the symptoms?
